Bridges Healthcare of Vermont is switching to an evaluation and consultation model
Bridges Intermediary Clinic
The goal of the Bridges Intermediary Clinic is to provide short-term outpatient psychiatry services and medication management so patients with worsening mental health problems can be seen by a provider while awaiting long-term care, providing stabilization and preventing mental health crises and emergency room visits.
The intermediary clinic is a short-term (2-5 appointment) service and patients must be on the waitlist for a long-term mental health provider (A psychiatrist or other prescriber who will manage their medications long-term).
This is a limited service, and patients must be referred by trusted referral sources. We are not able to accept self-referred patients at this time.
Bridges Neurodiversity Clinic
The Bridges Neurodiversity Clinic with OCA/MHS is housed at the Waterfront Vermont Center for Resiliency site in Burlington, VT.
The clinic will focus on assessment and management recommendations for diagnosis and problems related to Autism, ADHD and other neurodevelopmental differences. The clinic's focus on neurodiversity will emphasize inclusivity as well as embracing differences in brain development as opposed to only seeing these differences as pathological. The clinic will be problem-focused rather than disorder-focused - looking to enhance neurodiverse strengths while also helping to chart a child- and family-focused course to address the identified problem.

Things to know:
Hybrid
Bridges Healthcare can now see patients in person or via Zoom appointments. Telehealth requires appropriate technology (computer, laptop or smart phone, wireless internet or sufficient cell phone minutes for video conferencing, etc.)
A Bridge
Evaluations and consultations usually consist of 1 or 2 appointments.

Engagement
Bridges Healthcare requires active engagement. Please submit all requested paperwork promptly. We may also request telephone or video check ins, completion of online assessment measures, use of wellness apps, or other activities as assigned.
Teamwork
When accessing this service, it is also required that you have an established relationship with a Primary Care provider and/or other supportive professionals.
We will be requiring the completion of Release(s) of Information for these individuals.
Get set up for success
Bridges Healthcare Vermont collaborates with Headlamp Health which will gather your historical medical records, track your mood and behaviors, and build your awareness of how these feelings may contribute to your overall health and wellbeing.
