New Patients

Bridges Healthcare Vermont is only able to accept patients that are referred by select providers. We are unable to accept self-referred patients at this time.

After receiving your provider's referral our staff will contact you to collect more information. If we mutually agree your family is a good fit for Bridges Healthcare services, we will send you our intake paperwork and applicable assessment measures via our patient portal. Please complete the required paperwork and submit it ASAP to discuss an appointment time.

Please remember- Bridges Healthcare is an assessment and consultation service. When accessing Bridges clinics, it is required that you have an established relationship with a Primary Care Provider, and have a long-term prescriber if medications are recommended.

For our intermediary clinic- please make sure you are on the waitlist for continuing mental health support (i.e. Community Health Center, University of Vermont psychiatry, Green State Psychiatry, etc).
